Intro: First off you do this at your own risk, you will be running the same game twice which means you will be using a lot of system resources, therefore only attempt it if you have a powerful pc.
It will not work with all games, Crysis and Call of Juarez you can only run one instance of the game. Any game that allows 2 versions of it, both in windowed mode, might work.

Step 1: First off you will need to download the following,

Step 2: Install autohotkey. Now run the script by right clicking it and press 'Run Script'. Now the script is running you can see it in the taskbar.

Bonus Step for call of duty: Someone rightly pointed out that I forgot to say how to run call of duty in a window.
All you need to do is open up the call of duty folder then go to 'players\profiles\
profilename'. Once in here open up the 'config_mp' file. Search for fullscreen, then change the 1 to 0. You will need to do this for all players.
Step 3: Now Use Xpadder and make 2 controller settings (1 for each player)
Player 1 (These buttons will be the ones which autohotkey sends to the other window, you must use these buttons)

Step 4: Now run COD 4, create 2 profiles. Set the buttons for the players, you can set them how you like or you can copy them from the image below.

Step 5: Now start a lan game. Get player 1 to host then get player 2 to join. When you start you should both be able to walk around, if not alt and tab to the other window. If it still doesn't work right click the autohotkey icon and press reload script.
